To streamline your first visit with us, we’d like to invite you to fill out your new patient forms at home so you won’t have to arrive extra early. If it would be more convenient for you to arrive 15 minutes early to fill out paperwork in our waiting room, our front office has iPads that you can use to submit your forms. If you have had any dental X-rays taken within the last six months, you are welcome to have your previous dental practice email them to our team so we know what X-rays need to be taken at your first appointment.
If you have dental insurance, we’ll ask that you bring your insurance information with you to your first appointment. We accept many popular insurance plans, so if you are planning on using insurance, our team will be happy to check if your plan is in-network.
